Simon Hambidge, MD, PhD 
Chief Ambulatory Care OfficerCEO, Denver Community Health Services
Dr. Hambidge is Chief Ambulatory Care Officer at Denver Health and Chief Executive Officer of Denver Community Health Services (DCHS). A pediatrician by training, Dr. Hambidge was previously the Director of General Pediatrics for Community Health Services for 8 years. He is a Professor of Pediatrics at the University School of Medicine, and has practiced at Denver Health since 1997.
As DCHS Director, Dr. Hambidge oversees a network of 12 community health centers and 19 school-based clinics, which provide health care to over 199,000 patients, approximately one-fourth of the population of Denver. Denver Health’s patients include over half of the Medicaid patients in the city.
Dr. Hambidge still provides limited clinical care in the Primary Care Center at the Outpatient Medical Center. He has published numerous articles on topics such as vaccine safety and pediatric obesity.
Dr. Hambidge serves on the Board of Directors of the Colorado Community Health Network, the Community Health Association of Mountain and Plains States, Denver Health Medical Plan, Immunize Colorado, and HCPF’s Medical Services Board, which approves all rule changes for Medicaid in the state of Colorado (Governor appointment).
Ray Estacio, MD 
Medical Director for ACS QI and Research
Ray Estacio, MD is the Medical Director for ACS QI and Research whose responsibility is to improve healthcare delivery to Denver Health patients. Dr. Estacio a general internist and is a primary care physician at the Westside Family Health Center’s Adult Clinic. He graduated from Indiana University School of Medicine and completed General Internal Medicine training at the University of Colorado Health Sciences Center.
Dr. Estacio’s research projects focus on interventions to improve racial/ethnic disparities in diabetes and cardiovascular disease. His research career has included participation in a number of NIH and industry-sponsored prospective randomized clinical trials in diabetes, hypertension and CV disease over the past 20 years. This includes leadership roles in projects examining strategies to improving healthcare delivery which have been funded by NIDDK, NHLBI, AHRQ and American Diabetes Association. He also serves as Director of Community Health Programs for the Colorado Prevention Center (CPC) for the past three years. The CPC programs focus on promoting cardiovascular health through screening and education. These programs have been implemented in over 25 Colorado counties and are funded by a number of sources including local and national foundations and state and federal grants.
Rachel Everhart, MS, PhD 
Director of Research, Data and Analytics
Rachel directs the clinical and operational data management, analysis and reporting for the Ambulatory Care Services department at Denver Health. Dr. Everhart is involved in program evaluation for various grant-funded programs focused on improving the healthcare access, utilization, and quality of Denver’s medically underserved population. She gained experience working with and advocating for underserved populations through a year of service as an AmeriCorps VISTA. Dr. Everhart earned her PhD in Health Services Research from the University of Colorado at Denver and Health Sciences Center and holds a MS in Clinical Epidemiology and Health Services Research from Wake Forest University. She enjoys sharing the blend of urban and mountain amenities that Denver allows with her husband, two children, and Greater Swiss Mountain dog.
Sonja O’Leary, MD 
Chair of Ambulatory Pediatrics and Community Programs
Dr. O’Leary is Chair of Ambulatory Pediatrics and Community Programs and an Associate Professor of Pediatrics at the University of Colorado School of Medicine. She has over 20 years of experience as a clinician providing medical care to children and adolescents. She is currently serving as the Chair for Ambulatory Pediatrics and School and Community Programs and previously served as the Medical Director of Denver Health Pediatrics @ Denver Public Schools. She has worked to improve health care delivery and bring innovative care to the department and has a special interest in the intersection between school health and education. She recently served as the chair of the Council on School Health (COSH) for the American Academy of Pediatrics and has continued working with COSH most recently as editor of the upcoming 8th edition of the School Health Manual. As a native Spanish-speaker and a Colombian, she knows that different heritages, cultures and experiences bring important perspective to medicine. She also completed her certificate in Dissemination and Implementation Science through the University of Colorado Adult and Child Consortium for Health Outcomes Research and Delivery Science. Her work specifically focuses on implementation of pragmatic programs in community-based settings, promotion of health equity, and mixed- methods evaluations of implementation outcomes.
Deb Gardner, BSN, MSN 
Associate Chief Nursing Officer
Deb has a BSN, Cum Laude, from the University of Colorado and MSN, Honors, from Regis University. She has worked as nurse for more than 25 years in a variety of staff and leadership roles. Deb came to Denver health in 1999 as staff nurse in the hospital. Deb is a Board Certified Nurse Executive from American Nurses Credentialing Center.
Rebecca Hanratty, MD 
Chair of General Internal Medicine
Dr. Hanratty is the Chair of General Internal Medicine and is an Associate Professor of Medicine at the University of Colorado School of Medicine. She is a practicing physician at the Eastside Family Health Center. Dr. Hanratty received her undergraduate degree from the University of Notre Dame, her medical degree from the University of Colorado and did her Internal Medicine residency at the University of Colorado Health Sciences Center. Her research interests include health disparities, cardiovascular disease and chronic kidney disease.
Lara Penny, MD
Chair of Family Medicine
Dr. Lara Penny is the Chair of Family Medicine at Denver Health. Lara is a practicing Family Doc at the Montbello Family Health Center. She received her degree from Warren Alpert Medical School of Brown University and completed her Family Medicine Residency at UCSF San Francisco General Hospital. As a tireless advocate of Community Health, Dr. Penny is dedicated to social justice, healthcare equity and practice innovation.
Jen Grote 
Director Integrated Behavioral Health
Dr. Grote is the Director of Integrated Behavioral Health and is an Instructor of Medicine at the University of Colorado School of Medicine. She is a practicing psychologist at the Westwood Family Health Center. Dr. Grote received her undergraduate degree in psychology from Miami University, her master’s degree in counseling from Loyola University Chicago, and her doctorate degree in Counseling Psychology from the University of Denver. She completed her pre-doctoral internship and post-doctoral fellowship at Denver Health. Dr. Grote is in the RIHEL PCMAC fellowship program class of 2019 for leadership and advocacy. Her research and academic interests include provider resiliency, leadership, health policy and payment reform for behavioral health services. She enjoys spending time with her family in the Colorado outdoors and baking.
Haddas Lev, MS, MBA, RN

Haddas is the Associate Chief Operating Officer for Ambulatory Care Services. She earned a Bachelor of Science in Engineering from the University of Massachusetts, and a Master of Science in Operations Management and MBA from Rensselaer Polytechnic Institute. Haddas also hold a Bachelor of Science in Nursing from Metropolitan State University. She began her career in healthcare while completing a year of service as an AmeriCorps HealthCorps member working with the underserved and underinsured population at a Colorado Community Health Center. The focus of her work at Denver Health and Ambulatory Care Services has been on process improvement, facility design and construction, patient access, and growth and expansion strategy.
In her spare time, Haddas enjoys all of the wonderful outdoor activities that Colorado has to offer.
Duane Mata, DDS 
Chair of Dentistry, Denver Community Health Services
Dr. Mata is the Chair of Community Dental Services at Denver Health. Previously he was the dental team lead at Westside Family Health Center. Dr. Mata continues to provide clinical care two days a week at Denver Health and has practiced dentistry for over 17 years in private offices in the Northern Colorado and southeastern metro Denver areas. He is a true Denver native, born and raised. He received his undergraduate degree from the University of Denver and is a 2001 graduate of the University of Colorado School of Dentistry. Dr. Mata has supported and helped to integrate Dental services into the Women’s Care Clinic at Westside Family Health Center by partnering with the Delta Dental of Colorado Foundation CO-MDI program and the CDPHE Oral Health Partnership initiative. He oversees 6 community health dental clinics, 17 school-based clinics, and management and development of the new Outpatient Medical Center (OMC) Dental Clinic increasing our capacity for services with 30 new dental operatories set to open in spring 2020. Dr. Mata is married to his high school sweetheart and has three children.
